How to request an official certificate
What to include when you need a certificate, transcript, or registry extract from an office.
Certificate requests fail when names or dates do not match registry records. Copy data exactly from your ID and prior filings, and say which certificate type you need (residence, business standing, vital record copy, etc.).
Speed up processing
- State the legal purpose if the office asks for it on their checklist
- List fee payment method if you are mailing the request
- Provide a return address and phone for questions
